Congestive Heart Failure and Stroke: Lifesaving Education

Stroke is the third leading cause of death in the United States, even though 80 percent of strokes are preventable. Prevention, Identification and Treatment
Do you smoke and have difficulty catching your breath? Do you have a cough that won’t go away? Do you feel that you aren’t getting enough air or that you have a decreased ability to exercise? Join the medical director of the Center for Advanced Lung Therapies for a discussion focused on early identification of lung disease and areas for intervention. Cancer and Nutrition: Managing Eating During Treatment

Integrative and Innovative Approaches to Inflammatory Bowel Disease
The director of the Center for Inflammatory Bowel Disorders at CPMC will discuss alternative and integrative approaches to managing Crohn’s disease and ulcerative colitis. Kidney Wellness Program (FREE)
If you have signs of early kidney disease, this program provides the opportunity to meet with a registered dietitian for medical nutrition therapy and have ongoing health screenings, as well as to learn skills in an ongoing kidney education group. Patients receive individual attention and a personalized program that fits their lifestyle needs, helps them feel their best each day and prevents further progression.

SMOKING CESSATION


No Ifs, Ands or Butts
This intensive course includes techniques to manage withdrawal symptoms for smoking cessation and the physical and psychological side effects. It will focus on traditional skills and strategies, along with holistic approaches such as hypnosis, acupressure and mindfulness to cope with the loss of the smoking habit. Participants have the opportunity for personal medical consultations and supervision. Five-week program, six classes total. Initial one-on-one consultation with a therapist is required.

Asthma/COPD Management Program (FREE)
Led by an asthma/COPD nurse practitioner, this program aids patients with asthma or chronic obstructive pulmonary disease (COPD). Program components include

  • Current information on asthma and COPD; the causes of asthma and COPD; proper use of a peak flow meter, spacer, inhaler and medications; and how to effectively manage asthma and COPD through proper medication use
  • Assessment of environmental triggers
  • Development of an action plan to identify and manage asthma or COPD symptoms
  • Assistance in monitoring progress and improving asthma or COPD control
  • Help in obtaining a referral to an asthma/COPD specialist, if necessary

This program features personalized HIV medication education and case management from a certified nurse practitioner. The HIV clinical specialist

  • Provides education on HIV therapies as well as helps the patient and doctor decide which medications and treatments are best for the patient
  • Provides tips on how to manage medication-related side effects and HIV-related symptoms
  • Works with the patient’s doctor to enhance care
  • Coordinates specialty care
